Our Person-Centered Approach

    We collaborate with you and your Supports Coordinator to understand your goals, preferences, and strengths.

    We design a personalized plan aligned with your Individual Support Plan (ISP).

  • Deliver Supports

    Our trained caregivers and direct support professionals provide services in the home and within the community.

  • Review & Adjust

    We regularly monitor progress and adapt your plan as your needs evolve.

Quality & Compliance

We adhere to the ODP Provider Qualification Process to ensure the highest standards of care.

We actively participate in ODP monitoring and Quality Management (QM) initiatives to promote safety, accountability, and excellence in service delivery.

Eligibility & Enrollment

To qualify for ODP services, individuals must:

  • Have a documented diagnosis of an intellectual disability, autism, or a developmental disability
  • Demonstrate functional limitations in at least three major life areas (such as self-care, communication, learning, mobility, self-direction, or independent living)
  • Be eligible for Medical Assistance (Medicaid)
  • Meet ODP level-of-care requirements and waiver-specific criteria, including participation in one of the following programs:
    • Person/Family Directed Support (P/FDS) Waiver โ€“ Customized, family-driven services for individuals living at home with limited support needs
    • Adult Autism Waiver โ€“ Specialized programs for adults (ages 21+) with autism to enhance independence, employment, and social skills
    • Community Living Waiver โ€“ Flexible supports for adults who need consistent care but do not require 24-hour residential services

Once eligibility is established, a Supports Coordinator will work with the individual to develop an Individual Support Plan (ISP) that outlines goals, services, and desired outcomes.
